I don't think there is a real work around to not answer questions. My doc has introduced tablet devices for all his patients. But there has to be similar questionaire that has to be filled, bot on paper but in the device.

I belive there is on going effort already to bring improvement to the area of fitness tracking. I have heard of bunch of apps.

Agree with you about the security risk while spending good amount of time with papers. Helath Care menas PHI and that is a risk.

I agree self service via a mobile app with questions and answers or a kiosk would be great! So much time is spent on paper and it's a security risk!

Fitness tracking sounds like a cool concept, but it's simply not there yet. I have confidence it will improve over time with better apps, but it's going to take a robust third party developer community for that to happen.

Ugh! Yah, I think part of the patient experience that could use vast improvement is getting rid of some of the forms... At least offer patients the option to complete forms at their leisure at home online? Or perhaps the whole point of lengthy forms it to distract patients from the fact that they are waiting so long?

Simplify the user experience
I love the idea of focusing on the patient experience. When someone is ill they are most vulnerable and so many times they feel out of control. Technology can create a patient experience that is not as scary or cumbersome. I used a hand scanner that scanned my veins for identification at my last doctor visit it was very interesting and didn't require other verification beyond birthdate. It helped to simplify my visit but I still needed to answer lots of questions and do lots of paperwork.

Hopefully, with all the tracker data out there being collected, there will be faster, easier and better ways to conduct controlled randomized tests in natural experiments that help improve patient care incrementally by varying small changes over many many patients.
